When you walk into a new oral surgery practice, it’s important that you feel well-informed and supported at every step of your journey.
At Somerset Oral Surgery, we’re committed to providing you with the information you need to make the decisions that will ultimately impact your health. During your first visit, you’ll get to meet your surgeon and our team, receive a comprehensive oral exam, and learn about available treatment options.
Prior to your appointment
Before you arrive, please take a few moments to fill out these registration forms to expedite the check-in process. If you’re unable to, please arrive early to complete the necessary paperwork.
At your first appointment
At your first appointment, you’ll meet with Dr. Hecht or Gordon to discuss your concerns and treatment goals. Your doctor will review your health history, perform an oral exam to gain a comprehensive idea of your condition, and offer potential treatment options.
Although some treatments can be completed during your first visit, most will require a follow-up appointment. Should this be the case, we will schedule a date and time for your surgery and provide detailed instructions and information.
Please note: A parent or legal guardian must accompany any patients under 18 years of age to a consultation appointment.

- Your surgical referral slip and x-rays, if available
- A list of medications you're currently taking
- Insurance cards
- Valid identification (e.g. drivers' license)
- Form of payment
Insurance Policy
We are in network with the following insurance companies:

We will submit the claim to your insurance company on your behalf and provide you with an estimated out of pocket cost prior to treatment. Your insurance company will determine the amount of coverage and the difference in cost is your responsibility to pay.

We accept Visa, Mastercard, American Express, Debit Cards, Check, & Cash. We also accept insurance and financing.
Cancellations
We require at least 24 hours notice to avoid cancellation fees.
At Somerset Oral Surgery, we are committed to providing exceptional care at a reasonable rate. Unless other arrangements have been discussed prior to your surgery, we require payment at the time service is rendered.
What if I my dentist just took x-rays?
If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
What if my insurance doesn’t cover the entire cost of my procedure?
If your insurance doesn’t cover the full cost, we’ll work with you to review payment options. We offer flexible payment plans and financing options to help make your treatment affordable.
What is CareCredit and how can I apply?
CareCredit is a healthcare credit card designed to help patients finance medical expenses, including dental procedures, by providing flexible payment options. Apply here.
Do you accept payment through flexible spending accounts (FSAs) or health savings accounts (HSAs)?
Yes, we accept FSA and HSA cards for payments. Please check with your benefits provider to ensure the expenses are eligible under your plan.
What happens if I’m running late to my appointment?
If you’re running late, please call our office as soon as possible. We’ll do our best to accommodate you, but depending on our schedule, we may need to reschedule your appointment to avoid delays for other patients.
Can someone else bring me to my appointment if I can’t make it myself?
Yes, another adult can accompany you, but for patients under 18, a parent or legal guardian must be present. For surgical procedures, we recommend having a responsible adult drive you home and stay with you as you recover.
