Your First Visit

When you walk into a new oral surgery practice, it’s important that you feel well-informed and supported at every step of your journey.

At Somerset Oral Surgery, we’re committed to providing you with the information you need to make the decisions that will ultimately impact your health. During your first visit, you’ll get to meet your surgeon and our team, receive a comprehensive oral exam, and learn about available treatment options.

Before you arrive, please take a few moments to fill out these registration forms to expedite the check-in process. If you’re unable to, please arrive early to complete the necessary paperwork.

What to Bring

When you come to our practice for you first appointment, please bring the following:

  • Referral slip and X-rays, if applicable
  • A list of current medications
  • Medical/dental insurance information
  • Valid identification (e.g., driver's license)
  • Form of payment

Please note: A parent or legal guardian must accompany any patients under 18 years of age to a consultation appointment.

What to Expect

At your first appointment, you’ll meet with Drs. Hecht, Jacobus, or Gordon to discuss your concerns and treatment goals. Your doctor will review your health history, perform an oral exam to gain a comprehensive idea of your condition, and offer potential treatment options.

Although some treatments can be completed during your first visit, most will require a follow-up appointment. Should this be the case, we will schedule a date and time for your surgery and provide detailed instructions and information.

3D Imaging

At our practice, we use state-of-the-art technology—including CBCT scanning and imaging, intraoral scanners, digital X-rays and impressions, and guided implant placement—to accurately diagnose conditions and provide the most comprehensive and effective treatment possible, designed to meet your specific needs.

At Somerset Oral Surgery, we are committed to providing exceptional care at a reasonable rate. Unless other arrangements have been discussed prior to your surgery, we require payment at the time service is rendered.
